You probably like forms just as much as we do, but to participate in programs and move the process along quickly, we need accurate paperwork. You can find all Farm Service Agency (FSA), Natural Resources Conservation Service (NRCS), and Risk Management Agency (RMA) forms on USDA’s eForms database. Below are some common ones.

FSA Apply Now Packets

The Farm Service Agency (FSA) Apply Now Eligibility Packets include many of the eligibility forms needed to participate in USDA programs. Specific programs will also have a program application and may have additional program eligibility forms that need to be completed. Contact your local USDA Service Center for assistance.

Conservation Compliance

Income and Tax Information

Payment Enrollment Forms

USDA payments are generally directly deposited with your bank. To set up or change your direct deposit for USDA payments, you can fill out one or both of the following forms. Farm Service Agency customers can fill out the Payment Enrollment Form (SF-3881). Last updated: 02-2003.

Natural Resources Conservation customers can fill out the Direct Deposit Sign-Up Form (SF-1199A). Last updated: 04-2021.

Reporting Acreage

For crop insurance and disaster assistance, you will need to report crops planted. If you have crop insurance, this is done through your Approved Insurance Provider and then USDA gets the information from them. If you do not have crop insurance, you report crops planted with USDA using the Report of Acreage (FSA-578). This should be done for both spring- and fall-seeded crops. Last updated: 08-22-2019.

Reporting Losses

For other crop and livestock disaster recovery assistance and program benefits a Notice of Loss must be reported as soon the loss occurs or is apparent. How and when to file a Notice of Loss differs by programs. Use the Reporting Losses (CCC-576) for Notice of Loss. Last updated: 12-10-2019.

If your agricultural operation is impacted by a natural disaster, you should timely report losses to USDA.  For crops and/or livestock covered by federal crop insurance, you should immediately notify your local insurance provider.
